The colour of the top left pixel is taken as the border colour, and every edge is pulled in while it stays that colour. Transparent edges count as border whatever colour they claim to be.
A scan is never perfectly even, so raise the tolerance a little if a thin line survives. If the whole image is one colour there's nothing to keep, and the tool leaves it alone.

How do I remove the white space around an image?

Why is nothing being trimmed?
Usually because the border isn't as even as it looks. Scanner noise, a faint shadow or JPG compression can leave the margin very slightly different from pixel to pixel, so the edge never reads as one flat colour. Raising the tolerance tells it to treat close enough as the same, which is normally all it takes.
